| There are two sets of ratchet wheels with the teeth facing opposite, with a smooth bearing surface between them. 
The wrench, or handle, has a corresponding semi-circular bearing surface, and a spring-loaded pulling hook. It can be flipped over to reverse direction. This is a specialized ratchet form of hook spanner. |
